I, the undersigned parent/guardian of the above-named participant, hereby acknowledge and agree to the following terms and conditions:
1. Assumption of Risk
I understand that participation in sports activities involves inherent risks, including but not limited to physical injury, illness, and property damage. I acknowledge that my child's participation in the sports academy activities is voluntary and I assume full responsibility for any and all risks associated with such participation.
2. Medical Authorization
I authorize the sports academy staff to seek and provide necessary medical treatment in the event of an injury or illness to my child. I understand that reasonable efforts will be made to contact me in such circumstances.
3. Code of Conduct
I agree that my child will adhere to the sports academy's code of conduct, which includes but is not limited to respecting coaches, teammates, and opponents, following instructions, and displaying good sportsmanship at all times.
4. Release of Liability
I release and discharge the sports academy, its employees, coaches, agents, and representatives from any and all claims, liabilities, damages, or expenses arising out of or in connection with my child's participation in the sports academy activities, including but not limited to any injuries, accidents, or losses.
5. Insurance
I understand that the sports academy does not provide personal accident or medical insurance coverage for participants. It is my responsibility to ensure that my child has adequate insurance coverage.
6. Media Release
I grant the sports academy permission to use photographs, videos, or other media recordings of my child for promotional purposes, both online and offline.
7. Agreement to Policies
I have read and understood the sports academy's policies, rules, and regulations, and I agree to comply with them.
By signing below, I acknowledge that I have read, understood, and agreed to all the terms and conditions outlined in this waiver.
